Financing and Insurance
The La Jolla Center for Holistic Dentistry works with a number of major insurance carriers. As a courtesy, we are happy to file insurance claims for you. At the time of treatment, we will calculate your deductible, and determine the amount of coverage offered by your insurance. The total treatment cost however is due at the time services are provided. You will be directly reimbursed by your insurance company. The estimates made in a dental office may differ from the calculations of an insurance company. The fees listed for services in our practice may differ from those listed as “allowable” in your insurance carrier’s “UCR” fee schedule. Charges for dental treatments are directed to the patient, who is ultimately responsible for payment regardless of coverage under an insurance policy. Claims denied by insurance companies, or unpaid by insurance companies for longer than thirty days, are the responsibility of the patient. We work with our patients to determine coverage during the course of treatment planning, so that they can obtain the treatment they need in the most financially suitable manner. Our practice requires payment at the time services are rendered in our office. We want to make it as easy as possible for our patients to receive treatment, and work with you to make suitable arrangements. Forms of payment accepted at the La Jolla Center for Holistic Dentistry include cash, check, as well as Visa and MasterCard. We can also provide financing options when necessary, using a 3rd party financing company such as CareCredit, and can give details of the Dental Fee Plan program. I understand and agree that all services rendered by me, my dependents, or others assigned by me to my account are charged directly to me. I further understand I am personally responsible for payment. If I suspend or terminate care and treatment, any fees for services rendered will be immediately due and payable. Should the fees for the professional services not be paid in accordance with the provisions herein, reasonable attorney’s fees, plus applicable finance charges and disbursements, allowances and costs provided by law shall be included in the computation of the amount due. Finance charges can be applied to all past due amounts at the rate of 1.5% per month (18% annual rate). If the account is in default and turned over for collection, a collection fee will be added.
